For all name changes - marriage, divorce, etc., please update your name with the Social Security Administration first, and if possible bring proof of that change with you to your MDOT MVA appointment. www.ssa.gov

Public Assistance Records
Access and print your benefits letter with eligibility dates by logging into your Maryland Department of Human Services account at myMDTHINK Programs & Services Portal (maryland.gov)
Selective Service
Verify your selective service registration status at Verify Registration | Selective Service System : Selective Service System (sss.gov)
If you are a male between the ages of 18 and 26 you can register for Selective Service at Register | Selective Service System: Selective Service System (sss.gov)
Request a Status Information Letter if you are over 26 years old and failed to register for the Selective Service. A Status Information Letter is required for our agency and many other state and federal programs
There are a few cases when a man is exempt from the registration requirement. Men who are exempt do not have a Selective Service registration number and are in compliance with federal law
Social Security Card
Order a replacement social security card by logging into or creating your account at my Social Security | SSA
Apply for a social security card using Form SS-5-FS Application for the Social Security Card (ssa.gov)
Voter Registration Card
Search for your voter registration record on file
